About the medical form

The medical form is a helpful tool to give the guides and scout the help they need if they get sick or injured during the camp.

The medical form contains information about the guides or scouts health condition and also informers about any medications they use.

This is information that the parents usually will be asked for by the paramedics, but on a jamboree the parents are not always present.

Everyone that participates in the jamboree must fill out a medical form. This includes guides and scouts, leaders, staff and parents. Leaders and staff retain the forms them self, while the group leader keep the medical form for the guides and scout in their group. 

The medical form contains confidential information, and should be treated accordingly. It is the group leader’s responsibility to ensure that the medical forms are collected from all the guides and scouts in advance of the jamboree. Medical forms should be brought in contact with the health services / paramedics during the jamboree.

The group leader is requested to keep the medical forms in a sealed envelope in a safe place during the camp, and destruction of medical forms should be made immediately after returning home.
 

All information will be treated confidentially, and will be kept by the group leader for the duration of the jamboree. The medical form will be sent with the scout if they need to visit a doctor or first aider at or outside the camp. The group leader shall destroy the form when the group has returned home.
